The catheter is inserted through the working channel of the endoscope and via the endoscope you can see where you are. 20-50 ml of sterile saline is injected via the catheter deep into the lungs, after which as much fluid as possible is sucked up. The aspirated fluid is the sample in which a cytological examination can be performed to assess cell distribution.

Bacterial and fungal inoculation can also be performed on the liquid. The rest of the fluid is absorbed into the lungs.
